PRISM IV — Pediatric Risk of Mortality
RESEARCH / AUDIT TOOL ONLY. This calculator records and scores physiological variables for research purposes. It does not recommend treatment, investigations, or drug doses, and must not be used as the sole basis for patient management.
Record the most abnormal value within the first 4 (or 12) hours of PICU admission, per original PRISM IV methodology.
—
PRISM IV Physiology Score
pSOFA — Pediatric Sequential Organ Failure Assessment
RESEARCH / AUDIT TOOL ONLY. This calculator records organ dysfunction severity for research purposes. It does not recommend treatment, investigations, or drug doses, and must not be used as the sole basis for patient management.
Each organ system scored 0–4. Total pSOFA = sum of 6 systems.
PELOD-2 — Pediatric Logistic Organ Dysfunction (v2)
RESEARCH / AUDIT TOOL ONLY. Records organ dysfunction severity for research purposes only. Generates no treatment recommendation and must not be used as the sole basis for patient management.
Records the most abnormal value for each organ system.
RESEARCH / AUDIT TOOL ONLY. Records presence of consensus criteria for research classification only. Does not diagnose sepsis clinically, generates no treatment recommendation, and must not be used as the sole basis for patient management. Final clinical classification rests with the treating physician.
Records presence/absence of consensus criteria for research classification only.

Research Classification (SIRS / Sepsis / Septic Shock criteria met)
Renal — KDIGO Pediatric AKI / pRIFLE
RESEARCH / AUDIT TOOL ONLY. Records AKI staging for research purposes only. Generates no treatment recommendation and must not be used as the sole basis for patient management.
Enter baseline (or estimated baseline) creatinine and current creatinine, plus urine output, to stage by both KDIGO Pediatric and pRIFLE criteria.

Respiratory — PALICC ARDS / Oxygenation Index / OSI
RESEARCH / AUDIT TOOL ONLY. Records oxygenation indices and PALICC ARDS severity for research purposes only. Generates no treatment recommendation and must not be used as the sole basis for patient management.
Enter ventilation status and oxygenation values. OI requires PaO₂ (invasive); OSI uses SpO₂ as a non-invasive surrogate.
—
Oxygenation Index (OI)
—
Oxygen Saturation Index (OSI)
Pediatric Shock Assessment
RESEARCH / AUDIT TOOL ONLY. Records clinical parameters for research classification of shock severity only. Generates no treatment recommendation and must not be used as the sole basis for patient management. Final clinical classification rests with the treating physician.
